DIY Soccer Academy™ Media & Testimonial Release

DIY Soccer Academy™ values the privacy of our players and families. From time to time, we may wish to share player accomplishments, testimonials, photographs, videos, or other content to help celebrate player development and communicate the experiences of families participating in DIY Soccer Academy™ programs.
Participation in this Media & Testimonial Release is voluntary and is not required to purchase or participate in DIY Soccer Academy™ programs or services.
Parent/Guardian Authorization
I represent that I am the parent or legal guardian of the minor participant identified below and have the authority to provide this authorization.
By signing or electronically accepting this Media & Testimonial Release, I grant DIY Soccer Academy™ permission, subject to the selections and limitations below, to use approved photographs, video recordings, audio recordings, testimonials, statements, training accomplishments, and/or other media featuring or relating to my child.
Permitted Uses
If permission is granted, authorized content may be used for legitimate DIY Soccer Academy™ promotional, educational, and business purposes, including:
- DIY Soccer Academy™ websites and landing pages
- Social media accounts
- Email communications
- Digital advertisements
- Promotional graphics and videos
- Testimonials and success stories
- Online course or program materials
- Presentations and other promotional materials
DIY Soccer Academy™ may reasonably edit, crop, resize, format, or combine authorized content for these purposes, provided the content is not used in a materially misleading or defamatory manner.
Player Identification
To help protect youth participants' privacy, DIY Soccer Academy™ will generally seek to identify players publicly using limited information, such as a first name, first initial, or other non-identifying description, rather than publishing a child's full name.
A child's full name, precise location, school, team, contact information, or other sensitive identifying information will not be intentionally published as part of promotional content without additional appropriate authorization.
Testimonials
If I provide a testimonial, review, statement, or description of my child's experience or progress, I authorize DIY Soccer Academy™ to use the testimonial for the permitted purposes I select.
DIY Soccer Academy™ may make minor edits for spelling, grammar, formatting, or length, provided those edits do not materially change the meaning of the testimonial.
DIY Soccer Academy™ will not knowingly create or attribute a testimonial to me or my child that we did not provide.
Player Accomplishments and Results
If I voluntarily provide information regarding my child's soccer development or accomplishments, I authorize DIY Soccer Academy™ to share that information according to the permissions I select below.
Examples may include improvements in juggling, ball control, confidence, training consistency, or other soccer-development milestones.
Individual results vary, and publication of a player's experience does not represent a guarantee that other participants will achieve the same results.
No Compensation
Unless separately agreed to in writing, I understand that I will not receive payment, royalties, or other compensation for authorized uses of content under this Release.
Voluntary Participation
Granting media or testimonial permission is completely optional.
Declining permission will not affect my child's ability to participate in Foundation Academy™, the Foundation Academy™ 7-Day Access offer, Private Coaching Experience™, or other DIY Soccer Academy™ services.
Withdrawal of Permission
I may withdraw permission for future uses by contacting DIY Soccer Academy™ at the email address below.
DIY Soccer Academy™ will make reasonable efforts to discontinue future uses after receiving and processing the request.
I understand that withdrawal may not be able to reverse uses that were already lawfully published, distributed, printed, incorporated into existing materials, or otherwise completed before the request was processed.
Ownership
This Release does not transfer ownership of photographs, videos, testimonials, or other materials originally owned by the participant or parent/guardian.
It grants DIY Soccer Academy™ permission to use authorized materials according to this Release.
Parent/Guardian Acknowledgment
By signing or electronically accepting below, I acknowledge that:

I am the participant's parent or legal guardian or otherwise have legal authority to provide this permission.
I understand that participation is voluntary.
I understand how authorized content may be used.
I understand that I may contact DIY Soccer Academy™ to withdraw permission for future uses.
