Parent/Guardian Participation Waiver & Release

IMPORTANT — PLEASE READ CAREFULLY

This Parent/Guardian Participation Waiver & Release (“Agreement”) contains important provisions regarding participation in physical soccer training activities, assumption of risk, parental responsibilities, and the release of certain legal claims.

By signing or electronically accepting this Agreement, you acknowledge that you have read and understand its terms and are agreeing on behalf of yourself and, to the extent permitted by applicable law, the minor participant identified below.

2. Programs Covered

This Agreement applies to participation in soccer training, instruction, coaching, exercises, drills, challenges, demonstrations, and related activities provided by DIY Soccer Academy™, including participation through:

  • Foundation Academy™
  • Foundation Academy™ 7-Day Access
  • Private Coaching Experience™
  • Online training videos
  • Follow-along training sessions
  • Live or virtual coaching
  • Training challenges
  • Digital training programs and resources
  • Other DIY Soccer Academy™ soccer training activities

These activities are collectively referred to in this Agreement as the “Activities.”


3. Parent/Guardian Consent

I represent that I am the parent or legal guardian of the minor participant identified above, or otherwise have the legal authority necessary to authorize the participant's involvement in the Activities.

I voluntarily give permission for the participant to participate in DIY Soccer Academy™ Activities.

I understand that participation is voluntary and that I am responsible for determining whether the Activities are appropriate for the participant based on the participant's age, experience, abilities, physical condition, and circumstances.


4. Nature of the Activities

I understand that DIY Soccer Academy™ provides soccer training and instruction that may involve physical activities including, but not limited to:

running, jogging, jumping, balancing, changing direction, kicking, passing, receiving, dribbling, juggling, ball-control exercises, footwork, coordination exercises, agility movements, and other soccer-related activities.

Some Activities may require the participant to use a soccer ball, cones, walls, rebounders, goals, training equipment, furniture or household objects used as markers, or other equipment.

I understand that the nature and intensity of Activities may vary.


5. Acknowledgment of Risks

I understand that soccer training and physical activity involve inherent and other risks.

These risks may include, but are not limited to:

  • Slips, trips, and falls
  • Collisions with people, objects, walls, furniture, equipment, or other surroundings
  • Muscle strains
  • Sprains
  • Bruises
  • Cuts and abrasions
  • Joint injuries
  • Overuse injuries
  • Injuries caused by soccer balls or equipment
  • Injuries associated with running, jumping, kicking, turning, or changing direction
  • Injuries caused by unsuitable playing surfaces
  • Heat-related illness or dehydration
  • Injuries resulting from inadequate space or unsafe surroundings
  • Other serious injuries that may occur during physical activity

I understand that not every potential risk can be anticipated, identified, or eliminated.


6. Online and Remote Training

I understand that DIY Soccer Academy™ provides some or all instruction remotely through prerecorded videos, online content, live video sessions, or other digital methods.

DIY Soccer Academy™ coaches are not physically present with the participant during remote training.

As a result, DIY Soccer Academy™ and its coaches cannot:

  • Physically inspect the participant's training environment
  • Determine whether the playing surface is safe
  • Remove hazards from the training area
  • Physically supervise the participant
  • Verify that equipment is properly maintained
  • Physically intervene if an unsafe situation develops
  • Provide immediate physical assistance following an injury or emergency

I understand and accept these limitations.


7. Parent/Guardian Responsibility for a Safe Training Environment

I understand that I am responsible for ensuring that the participant has an appropriate and reasonably safe environment before beginning any Activity.

I agree to take reasonable precautions, including:

  • Providing adequate space for the Activity
  • Removing furniture, objects, debris, pets, or other potential hazards where appropriate
  • Ensuring that the playing surface is reasonably safe and suitable
  • Ensuring that appropriate footwear and clothing are worn
  • Ensuring soccer balls and other equipment are reasonably safe and appropriate
  • Providing adequate hydration and rest
  • Considering weather and temperature when training outdoors
  • Preventing participation in an area where the Activity could reasonably cause injury or property damage
  • Providing appropriate adult supervision based on the participant's age and abilities

If adequate space or safe conditions are not available, the participant should not perform the Activity.


8. Parent/Guardian Supervision

I understand that DIY Soccer Academy™ does not replace appropriate parental or adult supervision.

I am responsible for determining the level of supervision appropriate for my child based on the child's age, maturity, experience, abilities, training environment, and the Activity being performed.

I understand that younger players may require active adult supervision during training.


9. Participant Responsibility

I will instruct the participant to:

  • Follow coaching and safety instructions
  • Perform Activities within their abilities
  • Maintain reasonable control rather than attempting movements at unsafe speeds
  • Use equipment appropriately
  • Avoid intentionally dangerous behavior
  • Stop an Activity if conditions become unsafe
  • Inform a parent/guardian of pain, illness, injury, dizziness, or unusual discomfort

Participation should prioritize safe, controlled execution over speed or performance.


10. Physical Readiness & Medical Considerations

I am responsible for determining whether the participant is physically capable of safely participating in the Activities.

I understand that DIY Soccer Academy™ and its coaches do not provide medical advice, diagnosis, physical therapy, rehabilitation, or medical treatment.

If the participant has an injury, medical condition, physical limitation, or other circumstance that could affect safe participation, I understand that I should consult an appropriate qualified healthcare professional before permitting participation when appropriate.

The participant should stop exercising and seek appropriate assistance if the participant experiences pain, dizziness, difficulty breathing, faintness, illness, unusual discomfort, or other symptoms that make continued participation unsafe.

In the event of an emergency, appropriate emergency medical services should be contacted.


11. Assumption of Risk

I understand the nature of the Activities and acknowledge that participation involves inherent risks that cannot be completely eliminated even when reasonable precautions are taken.

On behalf of myself and, to the extent permitted by applicable law, the minor participant, I voluntarily assume the inherent risks associated with participation in the Activities.


12. Release and Waiver of Claims

This section is the one I would have an attorney customize for your state rather than simply publishing it verbatim.

A starting structure is:

To the fullest extent permitted by applicable law, I, on behalf of myself and, to the extent legally permitted, the minor participant, release and hold harmless DIY Soccer Academy™, its owners, officers, employees, coaches, contractors, agents, affiliates, successors, and assigns (“Released Parties”) from claims arising from the inherent risks of participation in the Activities or the ordinary negligence of the Released Parties, except to the extent such claims cannot lawfully be released or waived.

Nothing in this Agreement is intended to release or waive liability that cannot legally be released or waived under applicable law.


13. Property Damage

I understand that soccer training conducted inside or around a home or other property may result in damage to furniture, walls, windows, flooring, electronics, vehicles, landscaping, or other property.

I am responsible for selecting an appropriate training environment and taking reasonable precautions to protect people and property.

To the extent permitted by applicable law, DIY Soccer Academy™ is not responsible for property damage resulting from a participant's selection or use of a training area or equipment outside DIY Soccer Academy's control.


14. No Guarantee of Results

I understand that participation does not guarantee any particular result.

DIY Soccer Academy™ does not guarantee improvements in soccer ability, athletic performance, confidence, playing time, team selection, tryout outcomes, starting positions, competitive results, scholarships, recruitment opportunities, or other outcomes.

Individual development varies.


15. Compliance With Instructions

I understand that DIY Soccer Academy™ may provide safety instructions, equipment recommendations, space recommendations, and modifications to Activities.

I agree to encourage the participant to follow those instructions and understand that an Activity should not be attempted when it cannot be performed reasonably safely.


16. Media Permission Is Separate

I understand that this Agreement does not grant DIY Soccer Academy™ permission to use the participant's photograph, video, likeness, voice, name, or testimonial for advertising or promotional purposes.

Any such permission will be governed by a separate Media & Testimonial Release voluntarily authorized by the parent or guardian.


17. Severability

If any provision of this Agreement is determined to be invalid or unenforceable, the remaining provisions will continue in effect to the extent permitted by applicable law.


18. Governing Law

This Agreement will be governed by the laws of the State of Pennsylvania, except where applicable law requires otherwise.

Any venue, dispute-resolution, arbitration, or jurisdiction provisions should be established in coordination with DIY Soccer Academy's Terms & Conditions and reviewed by qualified legal counsel.


19. Parent/Guardian Acknowledgment

By signing or electronically accepting this Agreement, I acknowledge that:

  • I have carefully read this Agreement.
  • I understand that soccer training involves risks of physical injury.
  • I understand that DIY Soccer Academy™ provides remote training and cannot physically supervise the participant.
  • I am responsible for providing an appropriate training environment and supervision.
  • I voluntarily authorize the participant to participate.
  • I understand that this Agreement contains provisions affecting legal rights.
  • I am entering into this Agreement voluntarily.
